ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
General
All staff shall be fully trained to perform their role. Those involved in the retail sale of alcohol and door supervision will also be trained in the contents of the premises licence including times of operation, licensable activities and all conditions. Training shall be recorded in documentary form that will be available for inspection at the request at all reasonable times by an authorised officer from a relevant responsible authority. The records will be retained for at least 12months.
All staff shall be trained in the requirements of the Challenge 25 policies.
All staff shall be suitably trained in the operating procedures for refusing service to any person who is drunk or is under-age or appears to be under-age.
An incident book shall be maintained to record any activity of a violent, criminal or anti-social nature. The record will contain the time and date, the nature of the incident, the people involved, the action taken and details of the person responsible for the management of the premises at the time of the incident. The incident book shall be available for inspection at all reasonable times by an authorised officer of the relevant responsible authority. The records will be retained for at least 12 months.
All door staff shall be trained in the requirements of the Challenge 25 policies, the identification & recognition of drunks and the correct procedures to be followed when refusing service. Training shall be recorded in documentary form and those records will be available for inspection at the request at all reasonable times by an authorised officer from a relevant responsible authority.
The prevention of crime and disorder
There shall be a personal licence holder on duty on the premises at all times when the premises are authorised to sell alcohol.
There must be a documented security assessment, which must incorporate counter terrorism measures for the premises. The assessment shall be routinely reviewed and must be reviewed following the elevation of the change of the national threat level. All reviews shall be documented and be available upon request to any responsible authority.
The Designated Premise Supervisor must conduct a written risk assessment for the requirement of SIA licensed door supervisors.
When SIA licensed door supervisors are required the number of SIA licensed door supervisors employed shall be in accordance with the following ratio: A minimum of two (2) door supervisors will be employed for the first one hundred (100) customers and one door supervisor for every one hundred (100) thereafter.
The following details for each door supervisor will be contemporaneously entered into a register kept for that purpose:
Full name
SIA licence/badge number, and registration number of any accreditation scheme recognised by the Licensing Authority (including expiry date of that registration 6 or accreditation)
The date and time they began their duty
The date and time they completed their duty
The full details of any agency through which they have been allocated to work at the premises if appropriate
The register shall be available for inspection and copying at all reasonable times by an authorised officer of a responsible authority.
The register shall be kept at the premises at all times and be so maintained as to enable an authorised officer to establish the particulars of all door supervisors engaged at the premises during the period of not less than 12 months prior to the request.
A written drugs policy shall be in place and operated at the premises. It must detail the actions taken to minimise the opportunity to use or supply illegal substances within the premises. The policy must be made available for inspection and copying upon request by an authorised officer of a responsible authority.
The premise will have a written vulnerability and safeguarding policy, which all members of staff will be directed to read and sign that they have read the policy.
No binge drink promotions will take place.
No bottles or glasses shall be taken off the premises.
The premises shall support local crime prevention initiatives and clearly display crime prevention information or give advice that supports the prevention of crime and disorder.
Public safety
A CCTV system is fully compliant with the guidance contained in the Information Commissioner's Office {ICO) guidance document (www.informationcommissioner.gov.uk) regarding installation of CCTV is provided at the premises.
The CCTV equipment shall be maintained in good working order and continually record when licensable activity takes place and for a period of two hours afterwards.
The CCTV system shall cover all areas of the premises to which the public have access including any outside areas.
Images shall be retained for a minimum of 28 days
The CCTV system shall be capable of downloading images to a recognizable viewable format.
At all times the premises are open for business a member of staff shall be present who is capable of operating the CCTV system.
The CCTV system shall be capable of obtaining clear facial recognition images and a clear head and shoulders image of every person entering or leaving the premises.
If the CCTV equipment {Including any mobile units in use at the premises) breaks down the Premises Licence Holder shall ensure the Designated Premises Supervisor, or in his/her absence other responsible person, informs the Licensing Authority and the Police as soon as is reasonably practicable. This information shall be contemporaneously recorded in an incident report register and shall include the time, date and means this was done and to whom the information was reported. Equipment failures shall be repaired or replaced as soon as is reasonably practicable and without undue delay. The Licensing Authority and the Police shall be informed when faults are rectified.
The CCTV system covering any dedicated smoking area shall be fully operational.
Escape routes and exits, including external exits are maintained to ensure that they are not obstructed, in good order with non-slippery surfaces, free of trip hazards and clearly identified and a record is made of these checks,
All emergency exit doors will be easily operable and will not require the use of a key during the premises hours of operation. Safety checks are carried out before the premises is opened to the public and a record of these checks is made.
The fire brigade will be called at once to any outbreak of fire, however slight, and details recorded in a Fire Log Book. Access for emergency vehicles is kept clear and free of obstruction.
When disabled people are present, appropriate provision will be made for the safe evacuation of these patrons in the event of an emergency and these patrons wi 11 be made aware of these arrangements.
Adequate supply of first aid equipment and materials will be made available on the premises.
The number of patrons will be regularly monitored and a record made of these checks.
The prevention of public nuisance
Between 23:00 and 08:00 there will be total sound containment within the premises.
During opening hours suitable provision will be provided for the collection of smoking related litter.
Notices will be placed at all exit points requesting customers are considerate of those in the local area.
Venue management will ensure that appropriate measures are put in place to minimise the level of sound leaving the venue.
When live music or recorded music takes place inside the premises all doors and windows will be kept shut. Entrance/exit doors to automatically close once persons enter or leave the premises.
Venue will reduce the volume of amplified music after 2300.
Equipment to amplify music will only be positioned inside of the venue.
The protection of children from harm
The premises will operate the Challenge 25 age verification policy and will accept the following identification as proof of age: passport, driving licence or proof of age card bearing the PASS hologram.
Where films are classified by the relevant film classification body, children will only be admitted in accordance with the classification of the film (U, PG, 12A, 15, 18).
