ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
General - all four licensing objectives
All staff shall be fully trained to perform their role. Those involved in the retail sale of alcohol and door supervision will also be trained in the contents of the premises licence including times of operation, licensable activities and all conditions.
Training shall be recorded in documentary form that will be available for inspection at the request at all reasonable times by an authorised officer from a relevant responsible authority. The records will be retained for at least 12 months.
All staff shall be trained in the requirements of the Challenge 25 policies.
All staff shall be suitably trained in the operating procedures for refusing service to any person who is drunk or is under-age or appears to be under-age.
An incident book shall be maintained to record any activity of a violent, criminal or anti-social nature. The record will contain the time and date, the nature of the incident, the people involved, the action taken and details of the person responsible for the management of the premises at the time of the incident. The incident book shall be available for inspection at all reasonable times by an authorised officer of the relevant responsible authority. The records will be retained for at least 12 months.
All door staff shall be trained in the requirements of the Challenge 25 policies, the identification and recognition of drunks and the correct procedures to be followed when refusing service. Training shall be recorded in documentary form and those records will be available for inspection at the request at all reasonable times by an authorised officer from a relevant responsible authority.
The Premises Licence Holder or nominated person shall assess the type of event and the requirement for door staff.
If Door Staff are required all door staff shall be trained in the requirements of the Challenge 25 policies, the identification & recognition of drunks and the correct procedures to be followed when refusing service.
If Door Staff are required the number of door supervisors employed shall be in accordance with the following ratio: A minimum of two door supervisor will be employed for the first 100 customers and one door supervisor for every 100 thereafter.
If Door Staff are required the Premises Licence Holder and Designated Premises Supervisor shall ensure door supervisors are properly briefed and trained to manage queues in a safe and efficient manner.
If Door Staff are required door supervisors will be responsible for ensuring the safe, quiet and orderly dispersal of customers from the premises and the immediate vicinity of the premises.
If Door Staff are required all door supervisors will display their SIA badge in an approved method (i.e. arm display badge holder) at all times.
The prevention of crime and disorder
No binge drink promotions will take place.
No bottles or glasses shall be taken off the premises.
The premises shall support local crime prevention initiatives and clearly display crime prevention information or give advice that supports the prevention of crime and disorder.

Public safety
A CCTV system is fully compliant with the guidance contained in the Information Commissioner’s Office (ICO) guidance document (www.informationcommissioner.gov.uk <http://www.informationcommissioner.gov.uk>) regarding installation of CCTV is provided at the premises.
The CCTV equipment shall be maintained in good working order and continually record when licensable activity takes place and for a period of two hours afterwards.
The CCTV system shall cover all areas of the premises to which the public have access including any outside areas.
Images shall be retained for a minimum of 28 days.
The CCTV system shall be capable of downloading images to a recognisable viewable format.
At all times the premises are open for business a member of staff shall be present who is capable of operating the CCTV system.
The CCTV system shall be capable of obtaining clear facial recognition images and a clear head and shoulders image of every person entering or leaving the premises.
If the CCTV equipment (including any mobile units in use at the premises) breaks down the Premises Licence Holder shall ensure the Designated Premises Supervisor, or in his/her absence other responsible person, informs the Licensing Authority and the Police as soon as is reasonably practicable. This information shall be contemporaneously recorded in an incident report register and shall include the time, date and means this was done and to whom the information was reported.
Equipment failures shall be repaired or replaced as soon as is reasonably practicable and without undue delay. The Licensing Authority and the Police shall be informed when faults are rectified.
The CCTV system covering any dedicated smoking area shall be fully operational.
Escape routes and exits, including external exits are maintained to ensure that they are not obstructed, in good order with non-slippery surfaces, free of trip hazards and clearly identified and a record is made of these checks.
All emergency exit doors will be easily operable and will not require the use of a key during the premises hours of operation.
Safety checks are carried out before the premises is opened to the public and a record of these checks is made.
The fire brigade will be called at once to any outbreak of fire, however slight, and details recorded in a Fire Log Book.
Access for emergency vehicles is kept clear and free of obstruction.
When disabled people are present, appropriate provision will be made for the safe evacuation of these patrons in the event of an emergency and these patrons will be made aware of these arrangements.
Adequate supply of first aid equipment and materials will be made available on the premises.
The number of patrons will be regularly monitored and a record made of these checks.
The prevention of public nuisance
Notice will be placed at all exit points requesting customers are considerate of those in the local area.
Venue management will ensure that appropriate measures are put in place to minimise the level of sound leaving the venue.
When live music or recorded music takes place inside the premises all doors and windows will be kept shut. Entrance/exit doors to automatically close once persons enter or leave the premises.
Equipment to amplify music will only be positioned inside of the venue.
Between the hours of 23:00 and 08:00 noise emanating from Plymouth Argyle Green Taverners Supporters Bar must be inaudible at the nearest residential property.
The protection of children from harm
The premises will operate the Challenge 25 age verification policy and will accept the following identification as proof of age: passport, driving licence or proof of age card bearing the PASS hologram.
Where films are classified by the relevant film classification body, children will only be admitted in accordance with the classification of the film (U, PG, 12A, 15, 18).
