ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
· A CCTV system will be in operation at all times the premises is open for licensable activities.
· Images will be stored for minimum of 28 days and copies made available to police and authorised officers of the licensing authority on request.
· A fully working and maintained CCTV system capable of recording and storing images is installed on the premises. The system records at all times when the premises is open to the public and images are stored for a minimum of 28 days with date & time stamping. This covers all entry and exit points, stock location and checkouts.
· A person conversant with the retrieval and downloading of CCTV footage will be present on the premises at all times when open to the public.
· All recordings will be made available to an officer from a responsible authority upon reasonable request and in line with the provisions of the current Data Protection legislation.
· Security tags will be placed on high value sought after goods to prevent theft.
· CCTV equipment shall be kept in a secure environment.
· A daily operational log will be maintained, signed and dated to illustrate that the system has been checked and is compliant. If any failings were identified this is to be recorded on the CCTV Daily operational log form and a job raised using the in-house system to arrange repair.
· All staff involved in the retail of alcohol will be trained in relation to the law regarding its sale. This training will be completed prior to them being authorised to sell alcohol and refreshed every 6 months. A record of this training will be kept and maintained. A copy of the training material and associated forms shall be retained in the necessary folder as instructed by HO. The recipient of the training will sign to state that they have received and understood the training, and this will be dated. Training records will be kept on the premises at all times and made available to an officer from a responsible authority upon reasonable request.
· The premises will operate and adhere to an age verification scheme with the minimum standard of Challenge 25. Signs advertising the policy will be displayed in all areas where alcohol is for sale and throughout the premises.
· There will be till prompts for age restricted goods.
· Staff will be trained on the acceptable forms of ID that can be provided as proof of age, these include photo driving licence, Passport, PASS ID cards.
· An incident reporting system is in operation on site. Incidents are recorded electronically and are dated and referenced in sequential order. All incident reports are retained on site for a period of at least 12 months. All incident reports will be available upon request by an authorised officer.
· Incident reports will be used to record incidents such as -
o Alcohol related crimes on the premises
o Ejection of persons from the premises
o Alcohol related disorder
· The incident report shall hold as much detail as is possible to obtain such as details of the individual, details of the staff member reporting the incident, the time, date and details of the incident. Any CCTV or witness statements will be referenced.
· All refused sales will be reported in the refusal register
· Any visits by the authorities to request details or to report incidents/complaints will be recorded on the Authority form.
· Any complaints or concerns raised by customers or residence will be recorded on the Customer Contact Form
· A telephone number for the DPS will always be available on site even if the DPS is not present.
· No customer or staff member will be allowed to take from the premises an open bottle or vessel.
· No consuming of alcohol permitted in the store or on the site including the car park and yard.
